Registration Information
To ensure that your registration process goes as smoothly as possible, please use the following guidelines when checking in for both Inpatient and Outpatient services.
When checking in for both Inpatient and Outpatient Services you may be required to sign the Conditions of Admission Form, which is a consent for treatment and authorization to bill insurance.
If you are a Medicare recipient, you will be asked to complete a Medicare Secondary Questionnaire and receive your Medicare Rights Statement.
If you are a Champus recipient, you will be required to sign the Champus Rights Statement.
Remember to bring with you at the time of registration the following items:
All active insurance cards
Picture ID
Living Will, if you have one
